IU Health Medical Group is hiring! We are seeking a Nurse Practitioner or Physician Assistant to join our Primary Care Immediate Access team in Bloomington, IN at our Landmark Clinic!
Full-time
36 patient facing hours, 4 hrs admin
Tuesday & Thursday 12pm-7pm
Wednesday & Friday 9am start time. Flexible end time.
Occasional Saturdays required
100% outpatient
Quarterly on call service with RN Triage- avg. 4 times a year
Work directly with 1 physician and 5 APPs
Patient volume: ~88/week
Key Responsibilities:
This position will provide overflow urgent care for acute conditions, but also uncontrolled chronic disease, hospital follow-up care, Medicare Annual Wellness visits, and some new patient bridge to new Primary Care Provider.
This position will be expected to both care for urgent issues, but also fill care gaps in their chronic disease management thus helping the rest of the primary care team meet our quality goals (i.e. addressing preventative care, providing needed immunizations, confirming risk-based diagnoses, etc.)

ACNP, FNP, or PA certification required
